The olanzapine injection uses Medincell’s sustained-release technology to reduce administration burden, while the drug’s metabolic risks remain.
Key Takeaways
- Weltruza is FDA-approved as a once-monthly subcutaneous olanzapine injection for adults with schizophrenia.
- Olanzapine provides the pharmacological activity, Medincell’s technology controls release, and Teva develops and commercializes the product.
- No loading dose, supplemental oral olanzapine or mandatory post-injection observation is required. Prior oral tolerability must still be established.

What Happened
Teva announced U.S. Food and Drug Administration (FDA) approval of Weltruza on October 9, 2026. The product combines established olanzapine pharmacology with Medincell’s sustained-release technology, licensed to Teva as SteadyTeq. U.S. availability is expected in the coming weeks. (Teva approval announcement)
In Phase 3 SOLARIS, placebo-adjusted changes in Positive and Negative Syndrome Scale (PANSS) total scores at Week 8 were −9.75, −11.27 and −9.76 points for the 318 mg, 425 mg and 531 mg doses, respectively. All three differences were statistically significant; lower scores indicate less severe symptoms. (SOLARIS efficacy results)
Why It Matters
Monthly dosing offers an alternative when daily tablets are difficult to sustain. The more distinctive change is the absence of mandatory post-injection observation—a requirement that has constrained existing intramuscular olanzapine treatment because of post-injection delirium/sedation syndrome (PDSS).
Weltruza could make long-acting olanzapine easier to accommodate in outpatient care. It also expands Teva’s schizophrenia portfolio alongside Uzedy, its long-acting risperidone injection. (Fierce Pharma approval coverage)
Science Brief: Receptor Blockade, Sustained Delivery
Olanzapine antagonizes dopamine D₂ and serotonin 5-HT₂A receptors, among others. D₂ blockade contributes to its antipsychotic effects, including positive-symptom reduction, although the precise therapeutic mechanism remains incompletely understood.
After subcutaneous injection, solvent diffusion causes the copolymers to form a depot containing olanzapine, which is gradually released. The formulation extends exposure without introducing a new receptor target.

Simplified D₂-blockade model related to positive symptoms. Molecular shapes are schematic; the monthly delivery system is not shown.
The Main Risks
Weight gain, elevated blood glucose and lipid abnormalities remain relevant. No PDSS cases were reported in premarketing trials, but Weltruza’s PDSS risk is currently unknown. Removing mandatory observation does not establish zero risk.
Patients without previously demonstrated olanzapine tolerability must establish it with oral treatment before starting Weltruza. This differs from supplemental oral olanzapine after injection initiation, which is not required. (Weltruza prescribing information)
SOLARIS demonstrated efficacy versus placebo, not superiority over oral olanzapine or competing injections. Better treatment persistence and fewer hospitalizations will require additional evidence.
BP View
Weltruza’s initial opportunity is among patients for whom olanzapine is appropriate but daily dosing is difficult. Its delivery advantages may broaden use, while the metabolic profile will continue to influence treatment selection.
For Teva, the approval expands its long-acting psychiatric portfolio. For Medincell, a second approved product following Uzedy strengthens the platform’s development record, although each future formulation still needs its own validation.
The next test is whether easier administration translates into wider prescribing and sustained treatment. Coverage, clinic adoption, treatment persistence and post-launch safety will determine how much practical advantage becomes durable value.
